Purpose: The purpose of this assignment is for the student to plan and implement an individual intervention for the population served.
Knowledge: The student will use RT concepts, skills, and techniques identified by the literature and mentor as appropriate for 1:1 intervention in the specified setting. The student will gain experience in facilitating a synchronized intervention live with another person. You will develop an activity plan using the attached template. Your plan will include an icebreaker and a main activity geared towards meeting an identified goal, objective and outcome. Your activities must be relevant and creative and focus on the client’s treatment goals.
Task:
- For this assignment, you will plan and implement an individualized RT intervention with a client, volunteer, mentor or peer (this is at the discretion of the mentor).
- Using the format above, you will plan the intervention and submit the plan for approval to your mentor.
- Once approved, you will upload your plan to the blog post “Individual Plan & Implementation”.
- Next you will schedule and implement the intervention with whomever you and your mentor decide is appropriate. *Please note that if your mentor is not your client, nor is present at the actual implementation, you must record it and send the video to your mentor for review.
- Following the implementation, you will use the following evaluation form, to evaluate the success of the plan implementation. You should review this with your mentor and ask for feedback.
- Finally, upload this evaluation form to the same blog post “Individual Plan & Implementation”.
- Your mentor must comment that the forms uploaded are appropriate, complete and approved.
Criteria for Success: Rubric-
Individual Intervention Plan The student completes the individual intervention plan template attached and uploads to their blog.

Program Evaluation The student completes the program evaluation form according to the assignment description and uploads to the blog.

Mentor Approval The mentor has reviewed the assignments and implementation against the rubric and assignment description requirements and has given written approval that this assignment is complete.
