Part-Time, Fall & Spring
- Provide support to Infant and Toddler (ITC) of the Heartland staff and providers
- Assist the system manager, service coordinators and clinicians with assessment of family/consumer needs
- Facilitate and coordinate client intakes and developmental screenings and the development of the Individualized Family Service Plan
- Ensure compliance of data with local, state, and federal mandates
- Meet all requirements for Early Intervention-Targeted Case Management (EI-TCM) service coordination
