S.M.A.R.T Goals:
By the end of services, the group will identify at least 2 self care techniques in each self-care dimension (Physical, mental, social, spiritual, etc.)
- The group will identify multiple self care techniques in each self-care dimension
- The group will demonstrate an understanding of at least 2 self care technique
By the end of services the group will create an individualized self-care plan that incorporates at least 3 self-care techniques
- The group will identify current self care techniques and opportunity for improvement
- The group will brain storm strategies to over come road blocks
Agenda:

Lesson plan:
- Take roll call of all participants in attendance and mark those absent who are not attending.
- Lock doors to ensure confidentiality
- Introductions
- Each group member will introduce themselves with their name and the best piece of advice that they have received.
- Recap
- The facilitators will hand out a sheet with all previously talked about self-care techniques to enhance the memory of the participants.
- The group will have a lengthy discussion about the self-care dimensions and what self care techniques have been taught throughout the sessions.
- Group discussion questions:
- What self-care techniques do you remember from previous sessions?
- What self-care techniques do you use in your daily life?
- What self-care techniques do you think would not be useful and why?
- What self-care dimension (Physical, mental, social, spiritual) do you find most useful?
- How do you utilize the self-care techniques that have been taught?
- Group discussion questions:
- Facilitator: Make a list of the participants top 5 most used self-care techniques
- The facilitator will create
- Break
- Refreshments will be served
- Activity
- Each participant will fill out the chart below with their top 3 self-care techniques in each self-care dimension. The participants will then discuss their pros and cons of each technique and they will list 2 new techniques that they are wanting and willing to try.

- For the second part of the activity the participants will analyze the S.M.A.R.T goal below
- The group will then make S.M.A.R.T goals together to understand how to do it on their own for their homework.
- Debrief
- Each participant will receive the handout below and will be expected to fill out the chart and return it next session as a way to create a rough draft self-care plan. Before leaving the session each group member will fill out their S.M.A.R.T goals for themselves and then on their own time before next session they will fill in their activities, timelines, and how they did.
