Incident Reports

Incident Reports are legal documents of any event that may have or may have not caused injuries to a person or damage to area in recreation entity. These incidents can include an accident or injury within the staff or members, etc., damages to equipment, or any other disturbance, which will be completed through our facilities incident report document and kept at the front desk.

What Staff Members Should Know: 

  • Respond and report all accidents, injuries, or damages of employees, members, staff, and facility
  • Check for signatures
  • Submit report to supervisor immediately 
  • Complete entire form
